5th Grade Movie Night Form & Payment

FOOD and DRINK:

We will be offering each child one bag of popcorn, one small candy box/bag, and one drink free of charge.

We also recognize that the movie is during dinnertime, so we are offering each child the choice either to bring a meal from home or to purchase a meal that we will be ordering. We will be unable to offer an allergy-free option for anyone. If your child has a gluten or dairy allergy, please have your child bring something from home.  We sincerely apologize and appreciate your understanding.

We will be ordering burritos(Chicken or Vegetarian) from Chipotle at $11.00 per order. Each burrito includes cilantro-lime white rice, black beans, cheese, and fresh tomato salsa with a side of chips. These ordered meals will only be served to those children whose parents have paid for them in advance—ABSOLUTELY NO PAYMENTS WILL BE ACCEPTED OR MONEY COLLECTED DURING THE MOVIE NIGHT.

Please know that once you select an option to either bring food from home or purchase a meal, you will not be able to change your choice. Due to time constraints, we cannot accept changes to your choice.
